Note: If the application is submitted by a third party (other than the injured party), it is mandatory to provide an authorization letter with the signature authenticated by a public authority.

  • Loss adjuster's report / Expert report
  • Invoices, quotation, vehicle destruction certificate, or transfer of the vehicle registration
  • Photographs
  • Insurance policy
  • Information Centre certificate
  • Vehicle registration certificate
  • Payment receipt / Settlement receipt, provided it concerns a right of subrogation (recourse) by an insurance company
  • Criminal case file (in cases of Bodily Injury)
  • Medical certificates / Social security fund certificates / Medical expense receipts
  • Legalization documents of beneficiaries (in the event of a fatal accident)
